Thanks David. I looked up that document and the first thing I checked wasn't in there.
All my files with direct-vent combo oil boilers were getting rejected with "One of the values contained in the *.TSV is out of range". As per the ERS tech procedures s. 3.5.12.3 I was using the H2K default efficiency of 87%. My files for combo boilers connected to a chimney using 85% efficiency were all accepted.
Appendix C doesn't explicitly list combo boilers, but for a direct-vent furnace the range is 85-89%. A senior EA I spoke to confirmed my suspicion that I have to use 85%.
Another bug I encountered is that the mailbot still won't accept houses built in 2021, so I have to put in 2020. And then when the homeowner looks at their report and tells me the year built is wrong, I have to explain that NRCan hasn't figured out that their home could be more than 6 months old. I'd also like to tell the homeowner that many people at NRCan couldn't find their arse with both hands, but I keep that to myself...
